  • Attach the following along with the print out of the application form (if mentioned as required in the application)
    1. Original payment slip
    2. Certified copy of birth certificate & its English translation (if the original is not in English)
    3. Copy of the National Identiti Card (NIC)
    4. Certified copy of A\L result sheet (issued by the Dept. of Examinations) sheet or any other certificates.
    5. Certified copy of O\L result sheet (issued by the Dept. of Examinations) sheet or any other certificates (if mentioned as required in the application).
    6. Any other attachments mentioned in the downloaded application.
